Introduction

My experience with meldonium began in 2015. That was when a family friend who is a neurosurgeon recommended that I combine meldonium with Mexidol. At the time, I was struggling with high blood pressure of unknown origin. I have to admit that this combination helped me overcome the problem for good.

From that point on, I began looking into meldonium in greater depth and discussing it with sports physicians, bodybuilders and athletes. After nearly ten years of observing its effects, I can confidently say that this drug genuinely works in sport.

Meldonium, also known by the brand name Mildronate, is a compound developed in Latvia in the 1970s by Professor Ivars Kalviņš at the Latvian Institute of Organic Synthesis in Riga. It was originally intended for medical use, particularly in the treatment of heart disease, muscle ischemia and circulatory problems. The substance was designed to support the body during oxygen deprivation and metabolic stress by helping cells adapt, reducing fatty acid oxidation and increasing the use of glucose as an energy source.

For decades, meldonium was mainly popular in Eastern Europe and the former Soviet Union, where it was used both to treat patients with cardiovascular disease and to improve the overall physical capacity of older people. It remained largely unknown in the West until a high-profile doping scandal brought it into the spotlight.

In 2016, meldonium made international headlines after Maria Sharapova, the Russian tennis player, admitted that she had been using it for many years. When the World Anti-Doping Agency, WADA, added meldonium to its Prohibited List, the substance was detected in her system. Sharapova explained that she had been taking the medication for health reasons, including supporting heart function and recovery, but WADA's decision was clear.

Since then, meldonium has increasingly been viewed as a "drug for athletes", even though its effectiveness in improving athletic performance remains controversial within the medical community. However, people who have used meldonium often report a noticeable improvement in endurance and overall well-being. I am one of them and can confirm this based on my own experience.

In countries such as Russia, Lithuania, Belarus and Ukraine, meldonium is available in pharmacies and is commonly used by both amateur and professional athletes. Across much of Western Europe, although it is not formally approved as either a food supplement or a medicine, its popularity continues to grow. It is used by people taking part in obstacle course races such as Runmageddon, as well as triathlons, strength sports and bodybuilding - and for good reason.

How does meldonium work and what are its properties?

Meldonium works primarily by altering L-carnitine metabolism, which plays a key role in the oxidation of long-chain fatty acids within the mitochondria. It is a structural analogue of γ-butyrobetaine (GBB), the natural precursor of carnitine. By inhibiting the enzyme butyrobetaine hydroxylase, meldonium blocks the conversion of GBB into carnitine, thereby reducing intracellular carnitine levels.

Since carnitine is responsible for transporting fatty acids into the mitochondria, lower carnitine levels limit fatty acid oxidation and shift cellular metabolism towards glycolysis, meaning that glucose becomes the primary source of energy. This mechanism is particularly beneficial under low-oxygen conditions, as ATP production requires less oxygen and cells are less exposed to oxidative stress and damage.

As a result, meldonium improves the balance between oxygen supply and demand, supports tissue adaptation to ischemia, including in the heart muscle and brain, and dilates blood vessels, improving blood flow and cellular recovery. It also has neuroprotective effects, supporting cognitive function and protecting neurons from oxidative stress.

In simple terms, meldonium limits fatty acid oxidation while promoting more efficient glucose use and improving blood flow. This improves the body's energy efficiency and protects tissues under low-oxygen conditions.

The main effects associated with meldonium can be summarized as follows:

  1. Improved myocardial contractility
    Meldonium increases the strength of heart contractions, which is particularly important under ischemic conditions. It reduces the negative effects of oxygen deprivation and improves cardiovascular performance.
  2. Reduced infarct size
    Studies indicate that meldonium limits the extent of heart muscle necrosis during a heart attack, helping to preserve more functional cardiac tissue and improve the patient's prognosis.
  3. Prevention of cardiac arrhythmias
    Meldonium helps stabilize heart rhythm and reduce the risk of arrhythmias, including ventricular fibrillation. This allows the heart to adapt more safely to physical exertion.
  4. Increased exercise tolerance
    In people with heart failure and in athletes, meldonium improves the ability to sustain physical activity for longer, reduces fatigue and supports recovery.
  5. Lower blood pressure and reduced vascular resistance
    Meldonium dilates blood vessels, improving blood flow and reducing the workload on the heart. This effect also supports tissue oxygenation during exercise.
  6. Neuroprotective effects
    Meldonium improves microcirculation in the brain and protects neurons from damage caused by oxidative stress. Studies have also suggested its potential use in the treatment of neurodegenerative diseases such as Alzheimer's and Parkinson's disease.
  7. Improved glucose metabolism and protection against diabetic complications
    Meldonium improves glucose tolerance, lowers blood glucose levels without increasing insulin secretion and protects nerves from diabetes-related damage.
  8. Reduced risk of lactic acidosis during metformin treatment
    Meldonium limits excessive lactate accumulation, which is particularly beneficial for people taking metformin and improves the metabolic safety of treatment.
  9. Antioxidant effects and improved mitochondrial function
    Meldonium stimulates antioxidant enzymes, reduces oxidative stress and supports normal mitochondrial function. This allows cells to use energy more efficiently.
  10. Reduction of vasospasm
    Meldonium reduces excessive vasoconstriction caused by adrenaline or stress, improving blood supply to the organs and lowering the risk of tissue hypoxia.

Meldonium use in sport

Meldonium is often used to help manage high blood pressure during a steroid cycle. It can also be helpful after particularly demanding workouts, when fatigue and chills caused by exhaustion make it difficult to fall asleep. This was a particular problem for me, and meldonium proved effective. It helps the body recover faster, allowing for a higher training volume.

I have also noticed that during more demanding training blocks, meldonium helps maintain mental well-being. You simply feel more stable, less irritable and better able to cope with prolonged physical stress.

Meldonium is also commonly included in the routines of endurance athletes, including runners, cyclists and triathletes, who use it to improve recovery between training sessions. In combat sports, it helps athletes return to form more quickly after cutting weight and intense sparring sessions. I have also seen people use it after competitions or periods of intensive fat loss to restore energy levels and improve sleep quality.

In practice, meldonium acts somewhat like a shock absorber for the entire body. It reduces the effects of physical overload, supports circulation and helps restore the balance between exertion and recovery. This is why it is increasingly used not only during PED cycles, but also in natural training programs where endurance, focus and mental stability all matter.

Meldonium dosage

How long does meldonium last, and how often should it be taken?

Meldonium has a half-life of approximately 6-12 hours, so its effects last for most of the day, but not for a full 24 hours. In practice, this means it is best taken twice daily to maintain more stable blood levels. Its effects build gradually, as the body needs several days to shift towards a different pathway of energy production and fully benefit from meldonium's mechanism of action.

In my experience, the effects may not be noticeable after the first few doses. However, after 3-5 days, a distinct feeling of lightness usually appears, along with a noticeable improvement in exercise tolerance.

Oral dosing

  • General recommended dose: 500-1000 mg per day, divided into two doses taken in the morning and early afternoon.
    According to the package leaflet, the maximum recommended daily dose is 1000 mg. Some sources mention doses of up to 1500 mg, but this exceeds the recommended amount and may increase the risk of side effects.
  • For nootropic, fatigue-reducing or stress-relieving purposes: 500 mg per day, taken as a single dose or divided into two 250 mg doses.
    From my own experience, even a single 250 mg dose taken in the morning noticeably improves concentration, alertness and overall well-being.
  • Duration of use: 4-6 weeks, repeated 2-3 times per year.
    Although many athletes claim to have used meldonium for years without experiencing side effects, there is insufficient clinical evidence to confirm the safety of long-term use.

Intravenous dosing

  • Dose: 500-1000 mg per day, usually divided into two doses.
  • Duration: 10-14 days of injections, followed by continued oral treatment.
    This form is mainly used in clinical settings to treat acute conditions such as circulatory disorders or cardiac and cerebral hypoxia.

General notes

  • Timing: Do not take meldonium in the evening, as it may have a stimulating effect and make it more difficult to fall asleep.
  • Special groups: In older adults and patients with impaired kidney or liver function, the dose should be adjusted individually and reduced if necessary.

Meldonium and training

It is often said that meldonium should be taken immediately before exercise. However, in my experience, this approach is not optimal and may sometimes even have the opposite effect. The body needs some time to adjust its metabolism. For this reason, it is best to take meldonium 30-60 minutes before training if it is the first dose of the day.

Personally, I prefer taking 250 mg three times daily rather than a single 500 mg capsule. This provides a more consistent effect that feels milder. For me, 750 mg per day is more than enough. It produces a noticeable improvement in recovery, concentration and exercise capacity without causing excessive stimulation.

Side effects and contraindications of meldonium

I have based this section primarily on the package leaflet, as meldonium is first and foremost a medication. From my own observations, however, a distinctive type of headache can sometimes occur. It feels more like nervous tension than physical pain. Mild brain fog may also appear, particularly at doses above 1000 mg or when the entire daily dose is taken at once.

This is why I mentioned earlier that the full dose should not be taken all at once, even if the leaflet allows for it. It is better to divide it into smaller amounts, such as 250 mg per dose. Fortunately, most side effects are mild and temporary, but it is still important to be aware of them.

Most common side effects

Based on my own experience, as well as information from package leaflets and clinical studies, the potential side effects can be divided into several groups:

  • Allergic reactions - rash, hives, itching, skin redness and, occasionally, mild swelling. These are typical signs of an allergic reaction to a medication.
  • Gastrointestinal symptoms - nausea, vomiting, stomach pain, general discomfort and, in some people, a bitter taste in the mouth. These symptoms usually occur at higher doses or when meldonium is taken on an empty stomach.
  • Headaches and dizziness - headache, mild pressure in the head and occasional dizziness. In most people, these symptoms disappear after a few days of use.
  • Cardiovascular effects - occasional cases of an increased heart rate, known as tachycardia, or a drop in blood pressure. These effects are more common in people who already have heart problems.
  • Agitation or anxiety - meldonium can slightly stimulate the nervous system. In some people, it acts like a mild stimulant, particularly when taken later in the day.
  • General weakness or drowsiness - paradoxically, meldonium may sometimes have the opposite effect to what might be expected. This is usually caused by taking too high a dose.

Most users do not experience any side effects. As always, however, much depends on the body's individual response and the quality of the product.

In clinical trials, fewer than 15% of patients reported any adverse effects. This is a relatively low percentage, but it does not mean that meldonium should be used without common sense.

When to be cautious

There are situations in which it is better to avoid meldonium or consult a doctor before starting to use it:

  • Hypersensitivity to any of the ingredients - if a rash, itching or swelling appears after the first few doses, discontinue use.
  • Liver or kidney disease - the medication is metabolised in the liver and excreted by the kidneys. People with impaired liver or kidney function should exercise caution and may require a lower dose.
  • Pregnancy and breastfeeding - there is insufficient research, so meldonium should not be used.
  • Children - there is insufficient research, so meldonium should not be given to children.
  • Heart medications - particularly medications that lower blood pressure or affect heart rhythm. Meldonium may increase their effects when the substances are used together.

In generally healthy people who follow the recommended doses, meldonium is considered relatively safe. However, as with any substance that affects metabolism and the cardiovascular system, it is important to pay attention to how the body responds.

From my own experience, I can say one thing: meldonium is not a "magic booster" that works the same way for everyone. For some people, it provides excellent support for recovery and exercise capacity. For others, it may cause mild tension, agitation or even irritability.

This is why it is always best to begin with the lowest possible dose, such as 250 mg, and assess the body's response before increasing it.

Meldonium and the ketogenic diet

This question comes up frequently: can meldonium be combined with a ketogenic diet if its mechanism of action involves limiting the use of fatty acids as an energy source?

The metabolic conflict

A ketogenic diet shifts the body towards using fats and ketone bodies as its primary sources of energy. Meldonium, on the other hand, restricts the transport of long-chain fatty acids into the mitochondria and reduces their oxidation. As a result, the body relies more heavily on glucose, which is the exact opposite of the metabolic goal of a ketogenic diet.

When fully in ketosis, meldonium may therefore work against the purpose of the diet by inhibiting the process on which ketosis depends. In people who are well adapted to ketosis, it may cause a temporary decline in exercise capacity, including a feeling of heaviness, reduced energy or difficulty concentrating.

The body receives conflicting metabolic signals: on the one hand, it is expected to rely on fat for energy, while on the other, meldonium restricts this pathway.

From my observations, people following a strict ketogenic diet generally do not experience many benefits from meldonium. In some cases, the opposite occurs and they feel sluggish or have difficulty concentrating.

The effects tend to be considerably better in people following a low-carbohydrate diet that is not completely carbohydrate-free. In this case, meldonium may improve circulation, tissue oxygenation and recovery without disrupting metabolic balance.

What can you combine meldonium with?

The combinations below were either recommended to me by a doctor, as in the case of meldonium and Mexidol, or tested by me personally. I am not encouraging anyone to use medications in this way. I am simply describing how I used them, why I chose them and what effects I expected.

The key is understanding how meldonium works. It supports metabolism, tissue oxygenation and nervous system stability. This is why it can work synergistically with substances that support circulation, mitochondrial function and energy metabolism.

Meldonium + Mexidol

I first learned about this combination many years ago: 750 mg of meldonium per day, divided into three 250 mg doses, together with 125 mg of Mexidol three times daily.

In my case, the results were clear. My blood pressure stabilised, and the episodes of hypertension that had previously required treatment, including telmisartan, disappeared. Most importantly, my blood pressure remained within the normal range after I completed the treatment.

Why does this combination work? Mexidol, also known as emoxypine, has antioxidant and neuroprotective properties. It improves microcirculation and makes neurons more resistant to hypoxia. Meldonium, meanwhile, helps cells use oxygen more efficiently and protects the heart from excessive strain.

Together, they act on both the blood vessels and cellular metabolism - precisely the areas involved in the development of hypertension.

Meldonium + Glutathione + GHK-Cu

I currently use the following combination:

  • Meldonium: 500 mg per day
  • Glutathione: 100 mg subcutaneously per day
  • GHK-Cu: 1-1.5 mg subcutaneously per day

This combination works on several levels.

  • Meldonium supports circulation and metabolism, increasing the delivery of oxygen and nutrients to cells.
  • Glutathione neutralises free radicals and supports the liver's detoxification processes. This complements meldonium's effect on mitochondrial function.
  • GHK-Cu supports tissue repair and collagen production while also having anti-inflammatory properties. When combined with meldonium, the regenerative effect becomes more noticeable, with faster healing, better sleep quality and a more stable heart rate.

In my experience, this trio works particularly well for central nervous system fatigue, or CNS fatigue - the mental burnout that can develop after periods of intense stress or demanding training. The body genuinely seems to get a second wind.

Meldonium + Cardarine (GW501516)

While preparing for a Runmageddon race, I used this approach with an athlete, alternating between meldonium and Cardarine in phases lasting several weeks. Meldonium supported recovery and adaptation, while Cardarine improved endurance and VO2 max.

Why use them in rotation rather than together? Both substances affect metabolism, but in different ways. Meldonium shifts energy production towards glucose, while Cardarine promotes fatty acid oxidation. When used simultaneously, their effects may work against each other.

In practice, alternating between them proved more effective. We used a meldonium phase to support recovery and adaptation, followed by a Cardarine phase to increase exercise capacity.

The result was faster recovery, a better resting heart rate and greater tolerance for interval training.

Meldonium during anabolic steroid cycles

Meldonium can be a valuable addition during anabolic steroid cycles. It helps control blood pressure, which often rises due to water retention and increased blood volume. At the same time, it improves oxygen delivery to the muscles, allowing the body to adapt more quickly to progressively heavier training loads.

In practice, this can mean:

  • more stable heart rate and blood pressure,
  • better muscle oxygenation,
  • faster recovery between training sessions,
  • a lower risk of overtraining.

In the context of anabolic steroid cycles, meldonium acts somewhat like a safety buffer for the cardiovascular system. This approach was recommended to me by a sports physician who works with world-class bodybuilding competitors, and I have since confirmed its effectiveness many times through my own experience.

Meldonium is a versatile medication that fits into a range of strategies, from improving recovery and helping prevent hypertension to providing support during anabolic steroid cycles or endurance competitions.

The key is knowing what to combine it with, what not to combine it with and how to adjust the dose to the intended goal. In the right context, it is genuinely one of the most versatile medications for supporting metabolism and recovery.
